1. Back of Kentucky title must be signed and notarized.
2. Owner should bring his/her plate into the Clerk’s office and sign an Affidavit at the Clerk’s office, as to the disposition of said vehicle.
3. Taxes should be paid at that time, if taxes were not paid prior.
